How to use it

  • When: you want to give written consent or permission for one clear matter: allowing a person or company to use your premises or your name or image for a stated purpose, to collect something on your behalf, to register or apply for something that needs the owner's approval, or to accept a step affecting you.
  • Who signs: the person who gives the consent. The person who receives it can sign to acknowledge it.
  • Fill in everything in [square brackets] and strike out the options that do not apply. Describe the matter exactly: what is allowed, to whom, for how long and on what conditions.
  • Keep it narrow. The more exactly the consent is limited, the safer it is and the more likely the receiving office is to accept it.
  • The form matters: an office or authority may require its own form, a signature certified by a notary, a copy of your ID, or a translation. Ask in advance.
  • Tip: attach a copy of your ID if it is requested, write an end date, keep a signed copy and tell the other person you may withdraw consent in writing.
